OUR MISSION

To provide services and supports that have been designed in partnership with our Stakeholders (Residents, Visiting Professionals, Informal Supports, Allied Health Professionals, Partners, and Support Workers), to be person-centred and offer a holistic, supportive community. As a Service Provider, we hold ourselves accountable to deliver on the agreed individualised goals for each person.  We work to inspire their sense of belonging and active participation, helping individuals achieve their highest level of physical, emotional, and overall health and well-being.

We strive to encourage self and systemic advocacy, with a focus on choice and control as well as equal opportunity. This is underpinned by a commitment to respecting and accepting differences and preserving the human rights of all.

  • End-to-end Medication Administration and detailed recording and reporting systems.
  • Electronic progress notes, visitor and staff registration, incident reporting, outings register, and comprehensive online house calendar.

  • Developing personalised support plans.
  • Continuous Professional Development for Support Workers, striving to optimise care for our residents.

  • Running essential errands within the Resident’s scope of requests.

Connect Care takes a holistic approach to provide personalised, warm, homely support to Residents, creating an elevated lifestyle and way of living through community connection, a sense of belonging and empowering individuals to achieve their personal goals. Feedback from our Stakeholders reveals that Connect Care excels in comparison to the scope and quality of services provided by most SIL homes they have encountered. Connect Care is community orientated and our SIL Home Option creates a welcoming and open place.

We constantly review the resources available to ensure maximum quality and experienced Support Workers are readily available. The Connect Care team is highly supportive and engaged when it comes to reviewing and working with Stakeholders.
